Upon pressing the submit button below,

I agree that :
I understand the Privacy Policy pertaining to information that I provide on this form.

For all tuition assignments contracted between myself and the parent/student,
for the 1st 4-weeks lesson,
at the end of the 4-weeks lesson,
I will be paid by the parent 50% of the 4-weeks tuition fee
and subsequently I will be paid by the parent the full 100% tuition fees.

I will be paid by the parent at the end of every 4-weeks lesson.

For termination of contract before the end of the 4-weeks lesson,
payments to me will be pro-rated according to the above percentages.

If, at any time after the very first lesson,
the parent decided to cancel all future lessons,
I will be paid by the parent immmediately for all lessons
that are already conducted according on the above percentages.
