Upon pressing the submit button below,

I understand the Privacy Policy pertaining to information that I provide on this form.

I am the only person authorised to confirm the selection of a tutor.

For all tuition assignments contracted between myself and the tutor,
payment to the TUITION AGENCY is due at the end of the 2nd week lesson (BEFORE 4th week)
and payment to the TUTOR is due at the end of every 4-weeks lesson.

For the 1st 4-weeks lesson,
I agree to pay the TUITION AGENCY 50% of the 4-weeks tuition fee
and to pay the TUTOR 50% of the 4-weeks tuition fee.

Subsequently I will pay the tutor 100% of the tuition fees.

Tuition fees are due at the end of every 4-weeks lesson.

For termination of contract before the end of the 4-weeks lesson,
payments due from me will be pro-rated according to the above percentages.

If, at any time after the very first lesson,
I want to change tutor or cancel all future lessons,
I will pay immmediately for all lessons that are already conducted.
